Required software development,Website Development freelancer for Developper needed for the creation of an app job

We would like to have an app created for our company that can make our paper forms into digital versions that are easy to fill out, can be turned into pdf, and can be stored both on the app or sent in to the main account by email. The app would ideally work exactly like GoCanvas.

The forms would be filled out by electricians on their phones in the field and would be collected to make a report.

Ideally, the forms would look exactly or very similarily to the existing forms, which I will attach below.

There would be about 25 users on this app.
There is no set budget, but a quote for the work that would have to be done would be great.
