Required HTML,PHP freelancer for Contact Form in PHP with Validation & Email Sending job

I need somebody with skill in PHP . I have a project for my job and i need help to fix some problems. I have 2 files for each folder : 1) index and 2) action.
I need to receive the data (name , password, email, message) from index to my email via this action. I NEED A PERSON WHO CAN EXPLAIN ME HOW HE DID IT BECAUSE I WILL BE ASKED
