Required Microsoft Excel,sql freelancer for Create a small online database for collecting construction inspection report job

This is a report to inform the creation of a Database for input while out in the fields. Below are suggestions thus far:

Add a Request ID field for the Cityworks number
Add a Timestamp field for each address
Ask the City for input around what should be in the Recommendation and Next Steps fields.
Separate out the address so there is an entry field for each item City, State, Zip
