Required software development freelancer for External Contacts / Referrals Meeting Log (use Excel / Access, VBA, Userform, Advanced Filtering) job

I want to develop a Meeting Log to store information re: communications I or my team has had with external vendors. The information I want to capture is:
1. Company Name 2. Product Information 3. Initial Meeting Date 4. Type of Meeting (Call, In-Person, etc) 5. Team Members Involved 6. Referred by 7. Outcome
I think this could be somehow captured in either Excel, Access, or through a Sharepoint Form (preferred method is sharepoint so the list can be stored somewhere and actively updated). I assume that we can add some dynamic abilities to the Form, for example, the meeting date can be an actual calendar where they choose the date, the Type of Meeting can be a drop-down, the Team Members involved can potentially pull Team Member information from Active DIrectory or some other contact database we use (we are on Office 365). Looking forward to your offers and recommended approach.
