php: library to create customized user form for profile

PHP/MYSQL
My goal is to create a customized profile database for users, so admin can add customized field for user profile: ex:

username:text
age: number
smokes: select, selection, yes/no
spokenLangages, multiselection, french, spanish
....
The fields can be: text, textarea, select, radio, checkbox, multiSelect

Structure should be something like:
id, name, type (ENUM = radio, text, textarea...), label, placeholder, defaultValue, min, max, regexpattern...

Admin can add/edit new props any time

Imagine more an user profile where the admin will add fields and type of fields.
ex: admin add new prop called hobbies which are multiple checkboxes

cinema, reading, party, gaming, dancing, singing


The data should be searchable (ex: search for an user whose smoke=1 and spokenLangage is french and whose hobbies are cinema and reading.

I am 100% sure that was already developed dozen of times and I dont really feel like reinventing the wheel. So I am ok you use a ready to go library for that.
