File upload portal has stopped working (PHP, HTML)

We have a file upload portal on our website where users can upload their documents that are then stored on our server in designated folders. We inadvertently changed some of the code on our website, and since then the upload portal has stopped working. We are unsure whats wrong and have not been able to troubleshoot successfully.
