Moodle 3.1 database issue: check_mysql_file_format Error

We need to track down the source of a possible mysql issue. Moodle reports:

check_mysql_file_format Error reading environment data (14)[[unsupporteddbfileformat]]

on environment check with current version of moodle.

We are running in on old environment and cannot upgrade until we determince the nature of this issue.

mySQL 5.6
php 5.6
Cloudlinux 6.1
