Developer Needed to Create a Pricing Calculator Widget that can be Embedded into a SquareSpace Site

We are looking for a developer to create a widget for us that can be embedded into SquareSpace. This widget will have 3 independent fields, and base don those 3 fields a monthly cost, cost per case, and subscription type will come out as dependent variables. I already have the logic in an excel file, it just needs to be converted into an interactive widget that can be embedded into our website. View picture below for what the widget will look like.

This should be a pretty easy project considering that the logic is already done, happy to explain more in detail if you have questions.
