Developer Needed for Website Design

Looking to design a website with a dynamic registration process (Animal Registration)
Ex) Page 1: What animal do you have? (drop-down menu)
Page 2: Based on that animal different set of questions, so on and so forth

Progress Bar at the top of this process til completion.

At the end of the registration, I want the program to save the animals information to a database where we will harvest the data and spit out a unique Animal ID Code which can be searched for to look up animal information. On another area of the site, I want the user to be able to login and adjust their information previously inputted, add another animal, that animal be able to be looked up with their unique ID code, etc.

There will be more iterations that will go towards building out the site but this is the main bones of the site. Please let me know if you think youd be capable of building out a quality site with this format.
