Developer needed to make a INTERACTIVE HEALTHCARE DASHBOARD for patients in my small small practice.

I need a modern analytics template that I can use in my small healthcare business. I need it to monitor healthcare KPIs in a dynamic and interactive way for myself and patient so they can see their current information, lab results, and risk factors. I need patient statistics in real-time with each individual patient. So a template of sorts. The way I envision it,

Dashboard is colorful and includes several infographic style charts that are easy to read and can change when I put various values in the back office as I receive their lab values, screenings, and physicals. I also want certain values to indicate, if all values are at a particular point, to suggest an outcome. (similiar to if this then that.) for example, if the patient is over 50, has high LDL, LOW HDL, AND HIGH TRIGLYCERIDES, then I would like a animated box in the dashboard to come up suggesting my predetermined protocol and educational material.

This is a template ideally that i can use for many patients, as well as a design piece that looks nice and pleasing to the eyes.

I will share a some examples of the dashboard concept. Colorful, interactive, and i will need several topics/risk factors, graphs, info graphics and such. The lab report is almost identical to what i would like to have produced when i put in their values. I would like to have something similar.
