(Weekend Quick Job) Add a new pop-up on user login along w/ complete setup in existing application.

Now about the Job:

Backend is MS SQL Server (you wont need to touch it)
Middle tier APIs - C# (good to know C# to make integration easier)
UI code is Reactjs

When a certain (not all) users (with a particular affiliation saved in our DB) logs in, they would be presented with a pop-up to record their temperature and answer 2/3 yes/no questions. Save this data. Show the user a second pop-up based on their answer with one out of 3 types of text messages and send them a nice looking email with the same text.

We have a field where users save their affiliation. Currently they can type anything there. Is it possible for us to show all affiliations from above as type along/dropdown in this affiliation field. Users should be able to type anything if they do not select from this dropdown/type along.

We have plenty of pop-ups in our application so you can use that code. We also have email implemented in other parts of our application.

Our application support other languages, it is already setup so you have to make sure those work too.
