Web-form or MS Flow integration into Dynamics CRM on-Prem

I need a developer who can develop out webservice or use Flow/MS forms to connect to my on-prem version of dynamics.

This will be a simple form that on submission creates a new record in Dynamics and populates 10 fields.

We have a Microsoft data gateway already in place and working that syncs the Dynamics DB to the cloud.
