Responsive HTML5 Header Takeover Banner with Video - Template Creation

I am looking for a talented professional to create and develop a HTML5 Banner Ad unit that I will be using on my website. It is a Header Banner that will take over the top of the website and may include a video.

Deliverables: HTML5 Banner template for video to be served over standard banner.

The deliverable will be a template that I can easily swap images and video out of so that all my advertisers need to do is send me image files and videos and I will generate the ad and launch it for them. Template must be delivered in a format that I can edit, no proprietary coding or application.

Requirements:

- This needs to be a responsive HTML5 banner with compatible video player.
- It needs to work on both desktop and mobile browsers.
- Must contain proper code and compatibility to serve through Google Ad Manager (formally DFP).
- Must follow size requirements.
- Candidate must provide hourly rate and accurate estimate of hours to complete.
- Must be available for support for deployment and testing.
- Ad must be responsive, but use a 1600x400 size for larger screens, fluid.
- Must follow design references, compositions, and guides provided.

I can provide exact examples of the functionality Im looking for and will provide all creative assets. Prefer the template be provided in Adobe Animate, but Im open to other options.
