***URGENT***

Need a simple app developed.

It should generate an alert at desktop on specific time. Alerts generated on daily, weekly basis. The alert window also takes 3 inputs from the user: 1. Select an option from a drop down 2. Comment 3. Case info.

This app will run on 10 machines and should take input from all users and collate and show in a single file. You can select any language or technology to develop the app.
