Improve the UI (and possibly UX) of my Web App

I am working on a web app and using Vuetify as the CSS framework (which is built on Google Material). Its a large, multi-page questionnaire. I like the look, but it could use some improvements to make it easier to understand for the user. Ill give you access to the application so you can walk through the process. Im looking to receive a photoshop mockup of the improvements so I can code it.
