Need a fully functional sign up form in HTML/JS/CSS

I want a custom sign up form that can do the following:
Collect client details
Allow document upload
Allow client to sign agreement

Sign up form needs to be password protected

Client details need to be stored on Google Spreadsheet
Uploads should go to storage and only be accessible with authentication (e.g. Google Drive)
A signature needs to look good

I have taken screenshots of what the sign up form entails. I have also copied text into a word file, so there is no need to write anything but code.

Output will be:
1) Stand alone sign up form with a link that can be sent to clients
2) Code that can easily be embedded into existing websites
