MySQL Dev Needed to Fix Very High “Failed Attempts” On DB, Records Not Being Saved

I have a PHP file that is supposed to add entries into my localhost MySQL database. The issue is that although many of these entries are correctly entered, there are some that simply do not get entered. After trying to identify the culprit, I check PHPMyAdmin, and found that I have what seems to be a very high Failed Attempts amount. Here is the stats:

I am looking for a developer who can help me fix this issue. The DB structure is very simple, and the PHP MySQL queries are also very simple as well.
