Simple Android and iOS app for data collection

I am looking for a Experienced application developer who can develop both in Android and iOS. The app is quite simple with a splash screen on first page and then a home screen. There will be navigation menus from home screen towards a data capture form and "about us" page. A thank you page will be displayed after successful data transmission via email.

The app can be in Xamarin Forms, Xamarin Android and Xamarin iOS or in native Android and iOS. The decision is yours.

The form will be in Arabic and English, the user will select the language from a toggle button on the form. The form will capture the data, make an email body and send that to a designated email address. Once the email is sent successfully the customer will be notified that his message is received and he will be contacted shortly.

You must have good design and aesthetic sense to design attractive splash screen and form with very good look and feel and most importantly must have an experience in publishing apps on app stores. The app must meet the app store guidelines for successful publishing.

No GPS location tracking needed.
No user authentication required.
No user login/signups required.
Tablets (IPAD/Galaxy Tab) are not included in the scope.

Current minus-1 version will be supported for the mobile app
We will provide you with credentials and you will have to publish the app on app store as well as on google play store on our behalf.
We will provide you with logo and texts for splash screen.

Once the contract is finalized, i will provide you with wireframes to complete and deliver thoroughly TESTED apps.

The payment is made once, the apps are approved to get publish on app store and play store.

This is quite simple application with no database links, no web API's or services required.
